- Funeral expenses: Form 22, the death certificate, the deceased's e-Pehchan card or insurance number, the claimant's identity proof and bank details (as listed by ESIC).
- Sickness or maternity claims: the medical certificates issued by the ESI dispensary or hospital, and details of the employer.
- Employers or their staff: the 17-digit ESIC employer code and an authorisation letter.
